Comparison between biaxial and coaxial phacoemulsification in cases of cataract surgery in eyes with corneal opacities, using only the microscope light

Abstract Details
Purpose:
To compare the safety and efficacy of biaxial versus coaxial cataract surgery in eyes with corneal opacities, using only the microscope as source of light.
Setting:
Centro Oftalmologico Tlalnepantla, Mexico
Methods:
32 cases of cataract surgery in eyes with corneal opacity were operated by one surgeon with the use of Biaxial Phacoemulsification, with out an additional source of light, rather than the microscope light. Variables like incision size, capsulorhexis integrity, phacoemulsification time, amount of balance salt solution (BSS) utilized, intraocular (IOL) lens centration and intraoperative posterior capsule rupture were measured and compared with data of previous 17 cases operated by the same surgeon with Coaxial Phacoemulsification. Subjective variables were measured (from + to ++++ ) like accessibility, visibility and better maneuver performance
Results:
The Biaxial approach was statistically significative better than the Coaxial in incision size, capsulorhexis integrity, phacoemulsification time, amount of BSS used, postoperative IOL centration and rate of intraoperative posterior capsule rupture. Subjective variables also indicates improvement in the performance of Biaxial approach
Conclusions:
Biaxial Phacoemulsification is a safe and effective technique to perform Cataract Surgery in eyes with corneal opacities. Further work should be done to study Biaxial approach with endoilumination in comparison to Coaxial
