Steeper anterior lens curvature as an indicator of zonular weakness

Purpose:
To identify a novel marker of zonular weakness.
Setting:
Refractive clinic
Methods:
Consecutive patients undergoing femto assisted cataract surgery were evaluated for increases in anterior capsular curvature and increased distance between iris plane and the apex of the anterior capsule and the presence of intraoperative zonular weakness.
Results:
8 eyes were identified who had an increase greater than 20% over the mean anterior capsular curvature and/or an increase of at least 0.2 mm of mean distance from iris plane to apex of anterior capsule. All of them were found to have significant intraoperative diffuse zonular weakness resulting in the need of a CTR in all and 2/8 required suture fixation to the sclera. 1/302 eyes in the control group required a CTR.
Conclusions:
An increase in the anterior capsular curvature and in the distance of the anterior capsular apex from the iris plane are indicators of an increased spherical shape of the natural lens due to zonular weakness even in older patients. Special caution should be applied to those patients.
