Marfan syndrome: diagnosis and management

Abstract Details
Purpose:
To recall the ocular manifestations and management of Marfan syndrome in children and juvenile population, in order to prevent ocular complications for a good visual outcome.
Setting:
Cheikh Zaid university hospital, Department of ophthalmology, Rabat, Morocco.
Methods:
A 12 years old patient was admitted in December 2017 for reduced visual acuity in both eyes 3 weeks prior to the consultation. She underwent a complete general and ophthalmic examination, which included visual acuity testing, Goldmann applanation tonometry, angle evaluation and slit lamp examination before and after dilation. The patient underwent lensectomy with lens implantation and a postoperative orthoptic treatment.
Results:
We founded a big sized height child with arachnodactylia and a mitral regurgitation, Visual acuity was at 0,3 (LogMAR) in both eyes, the ophthalmic examination revealed myopia, inferior subluxation of the lens, intraocular pressure was normal, and the fundus examination revealed a peripheric chorioretinal atrophy in both eyes. The patient underwent surgical removal of the dislocated lens with an aphakic artisan lens implantation. After surgery, the best corrected visual acuity (LogMAR) was at 0,1.
Conclusions:
Myopia and lens subluxation are characteristic ocular findings in children and teenagers with Marfan syndrome. The management can be challenging, therefore monitoring these patients closely is mandatory for a proper orthoptic and surgical treatment in order to prevent the complications.
