Abstract Details

Purpose:

Negative moments of cortex removal are: possibility of damage of lens capsule and iris, additional stress affecting corneal tunnel incision and paracentesis, excessive influence of irrigation on corneal endothelium. Thus, the purpose of our work was the study of effectiveness of viscodissection method for optimization of phacoemulsification (PE) of senile cataract.

Setting:

The work was performed in the Khabarovsk branch of the S.Fyodorov Eye Microsurgery Federal State Institution.

Methods:

245 patients (245 eyes) who underwent PE (Centurion, Alcon) of senile cataract with IOL implantation were under observation. One surgeon did all operations. All eyes were divided into 2 groups. In 123 eyes of the main group, after performing capsulorhexis we separated cortex from lens capsule using OVD (viscodissection) with simultaneous nucleus rotation, then standardly executed following surgical steps. In 122 eyes of control group PE was carried out according to standard technology (with hydro dissection). The following parameters - duration of operation, BSS volume and presence of residual cortex were studied.

Results:

All operations passed uneventfully. In 35 eyes (28.4%) of the main group full spontaneous removal of cortex during epinucleus aspiration took place. At the same time, different amount of residual cortex was observed in all eyes of the control group. Mean operation time in the main group made 5.7±0.5 minutes in comparison with 6.8±0.41 minutes in the control group. BSS volume delivered into eyes in the control group was 1.5 times more than in the main.

Conclusions:

Method of viscodissection applied at PE of senile cataract allowed occurring full simultaneous cortex removal during epinucleus aspiration in 28.4% of eyes in the main group. Duration of operation and volume of spent BSS were also lower in the main group.
