Purpose:
To develop a safe method of performing primary posterior capsulorhexis after the IOL implantation
Setting:
Eximer Eye Center, Moscow, Russia
Methods:
The total of 34 patients (40 eyes) were examined and treated. Our method consists in performing the primary posterior capsulorhexis after the multifocal IOL implantation and involves the use of two types of viscoelastic material. The viscoelastic with higher molecular mass (Healon GV) is put on top of the IOL and the viscoelastic with lower molecular mass (Viscoat) is introduced underneath the surface of the IOL. The difference of molecular masses pushes back the anterior hyaloid membrane ensuring safe manipulation with posterior capsule. The rest of the procedure is done in the classic way.
Results:
No significant intra- and postoperative complications were noted. An increase in UCVA was from 0.13±0.11 to 0.85±0.21 for distance and 0.77±0.21 for near. BCVA raised from 0.25±0.20 to 0.93±0.13 in the distance and 0.83±0.21 for near vision 6 months after the surgery (p<0.05). Reduction of spherical and cylindrical refraction component was shown.
Conclusions:
The data obtained indicates that the proposed method is safe and effective. It provides a surgeon with stable posterior capsule easy to manipulate with. To determine indications and contraindications it is necessary to conduct a comparative study with patients with preserved posterior capsule after the multifocal IOL implantation.
