Topical anesthesia in paediatric cataract surgery

Abstract Details

Purpose:

Despite general anaesthesia is the choice for paediatric cataract surgery, our aim is to demonstrate that is feasible to perform topical anesthesia with safety and comfort for both surgeon and patient in cooperative selected cases.

Setting:

Clínica de Olhos Menna Barreto , Porto Alegre - RS Brasil

Methods:

We evaluated the transoperative and results of paediatric cataract surgery performed using topical anesthesia in children aged 5- 9 years carried out between 2008 and 2015. This study cases included 6 eyes from 5 children (4 traumatic and 2 developmental). Topical Anesthesia was initiated upon arrival in the operating room. Four drops of Bupivacaine Hydrochloride 0,75% (Marcaine) were given 2 minutes apart. The practice of sedation (Midazolan sublingual ) should be used to cover only anxiety. All subjects underwent phacoassisted lens aspiration and primary posterior intraocular lens (IOL) implantation. No primary posterior capsulectomy was performed.

Results:

No complications that could be attributed to the anesthesia were encountered. The level of all trans and postoperative comfort of the patients were great. No requirement for postoperative analgesia was noted.

Conclusions:

The evaluation of the possibility of the children initiates at the beginning of the first consult. With a careful selection of cases, topical anesthesia can be a great alternative option for cataract surgery in paediatric patients. Good psychological preparation of the patient, the presence of the parents -from the first consulting room to operating room- and getting acquainted with the staff are mandatory to ensure that the children feel safe and cooperate during the procedure.
