Evaluation of anterior lens capsule after implantation of IOLs with different designs in patients with PEX

Abstract Details
Purpose:
To study the condition of the anterior lens capsule (ALC) in patients with pseudoexfoliation syndrome (PEX) after implantation of the IOLs with different designs.
Setting:
S. Fyodorov Eye Microsurgery Federal State Institution, Novosibirsk Branch, Russia
Methods:
OCT-study (RTVue XR Avanti, «Optovue») of the anterior segment was conducted in 30 patients with PEX after phacoemulsification with implantation of acrylic IOLs with different designs: planar (15 cases) and step-vaulted (15 cases).
Results:
In the early postop period ALC-IOL contact was not observed in all cases. In the long-term period there were revealed differences in the relationship of ALC and IOL. In cases of planar IOL implantation the ALC after polishing adjoined the IOL surface without any visible change in its morphology. In cases without ALC peeling there were opacification and thickening of the ALC complex due to proliferation of myofibroblasts. In patients with step-vaulted IOL design there was observed a free ALC edge with a hyperreflective layer of subcapsular epithelium, which did not realize its potential for metaplasia.
Conclusions:
The anterior capsule changes in patients with PEX after implantation of planar monolithic IOLs without peeling were expressed in epithelial metaplasia and manifested by opacification and thickening of the ALC complex with various degrees of the anterior capsule contracture. There were not observed pathological deformation of the ALC in cases of intraoperative anterior capsule polishing as well as in cases using IOLs, which design excludes ALC-IOLcontact,
