Purpose:
Evaluate two cases with different methods to overcome unexpected refractive surprise after 10 years of cataract surgery
Setting:
Jakarta Eye Center Hospital, Indonesia
Methods:
1st case: Male, 40 y.o, pseudophakic RE with history of cataract surgery 13 years at local hospital. BCVA reached 1.0 with S-6.75 and C-0.75. IOL was clear with good centration and fibrotic Soemmering's ring of anterior capsule.The patient chose the IOL exchange as the management of refractive surprise.
2nd case: Male, 36 y.o, history of high myopia and longstanding retinal detachment on his fellow eye, had cataract surgery 10 years ago. BCVA was 0.25 with S-16.00 and C-2.00. The IOL was decentered but stable, suspicious of tear at posterior capsule. Piggyback IOL using iris clip anterior chamber IOL was performed.
Results:
Although the surgery was quite challenging, best corrected vision for case 1 could reach 1.0 on day 1 and remain stable for 1 month with correction of S-0.50 and C-1.00. Meanwhile in case 2, best corrected vision was 0.3 with S-1.00 and C-2.00 because of history of high myopia.
Conclusions:
Exchange IOL and piggyback IOL could be the safe and effective choice for treating unexpected refractive error after cataract surgery with satisfying result.
