Comparative analysis of two clinical diagnostic methods of the corneoscleral geometry

Abstract Details

Purpose:

The measurement of corneo-scleral geometry became highly relevant in recent years due to the rise of scleral contact lenses. The main objective of this study is to compare for the first time two diagnostic technologies of corneal scleral geometry for clinical use.

Setting:

Methods:

In a prospective, non-randomized and comparative study, 40 healthy eyes from 20 patients were analysed. A corneoscleral topography was performed with the Fourier domain profilometry based system Eye Surface Profiler (ESP) (Eaglet-Eye, Houten, the Netherlands) and with the Scheimpflug camera-based system Pentacam (Oculus GmbH, Wetzlar, Germany). Different geometric parameters of the corneo-scleral structure have been recorded. The Kolmogorov-Smirnov test was used to assess the normality of the data distribution. For each variable, the mean and standard deviation have been defined. Using non-parametric statistics, the differences in the measurements of the corneo-scleral geometry obtained with ESP and PENTACAM have been analysed.

Results:

The analysis found a statistically significant difference between the cornea Best Fit Sphere (BFS) of PENTACAM and the inner BFS of ESP (p = 0.018), the scleral BFS of PENTACAM and the outer BFS of ESP (p = 0.028), as well as between average sagittal height of PENTACAM and ESP (p = 0.028) at 15 mm of chord. No statistically significant differences were detected between the mean sagittal height of PENTACAM and ESP (p = 0.068) at 13 mm of chord. Bland-Altman analysis provided clinically relevant ranges of agreement for scleral corneal fit curve as well as sagittal height.

Conclusions:

The measurements of the corneo scleral geometry obtained with the Eye Surface Profiler (ESP) and with the PENTACAM are not interchangeable. The Eye Surface Profiler tends to provide a lower sagittal height than that obtained with the PENTACAM.
