Abstract Details

Purpose:

To find the most sensitive existing parameters to diagnose and distinguish normal cornea with forme fruste Keratoconus (FFKC) and early-stage keratoconus (EKC) in Chinese patients.

Setting:

Wenzhou Medical University Eye Hospital

Methods:

Patients were divided according to clinical diagnosis and Belin ABCD grading system from Pentacam HR: normal group, including 1 eye randomly selected from 200 patients who has performed refractive surgery with more than 1-year stable follow-up, FFKC group, including 1 eye selected from 58 keratoconus(KC) patients which was in stage 0 of Belin system, and EKC group, including 1 eye randomly selected from 89 KC patients that in less than stage 1 of Belin system. Receiver operating characteristic (ROC) curve analysis was performed in over 100 parameters from Pentacam HR and Corvis ST to distinguish normal and KC eyes.

Results:

The area under the curve (AUC) ROC values in the comparison of the normal group and EKC were 0.959 (BAD-D) and 0.948 (TBI). The cutting value of 1.94 for BAD-D provided a sensitivity of 86.96% and a specificity of 98.02%. The AUC ROC values in the comparison of the normal groups and FFKC were 0.816 (Max InverseRadius), 0.787 (BAD-D) and 0.779 (TBI).

Conclusions:

The BAD-D and TBI exhibited outstanding distinguishing ability in eyes with EKC from normal eyes and with a relatively lower ability with FFKC.
