One-year visual outcome of pinhole sulcus implant for the correction of irregular corneal astigmatism and iris damage

Abstract Details
Purpose:
To evaluate the visual outcomes of patients who had implantation of an intraocular pinhole device (Xtrafocus) for irregular corneal astigmatism or iris injury.
Setting:
Corneo-plastic unit, Queen Victoria Hospital, East Grinstead, UK.
Methods:
This is a retrospective case series. Electronic case records were search for the following outcomes: uncorrected visual acuities (UCVA) and best-corrected visual acuities (BCVA), automatic or manifest refraction and spherical equivalent (SE). All main outcomes were evaluated preoperatively and at 1, 3, 6, 9 and 12 months postoperatively, with all complications reported.
Results:
Thirteen eyes of 13 patients with mean age 53.8 (range 27-81) years were included. Mean UCVA improved from 0.67 (range 0.1-1.22) logMAR pre-operatively, to 0.45 (range 0-1.3) logMAR at 1-month, p-value =0.0005; mean BCVA improved from 0.46 (range 0-1) logMAR pre-operatively, to 0.4 (range 0-0.8) logMAR at 1-month, p-value= 0.38. Vision remained stable for the following months in ten patients (77.0%). Six patients (46.1%) expressed satisfactory visual outcome. Complications in 5 patients (38.5%): one required iris reconstruction with no worsening of BCVA; one lens became subluxed, one developed post-op cystoid macular oedema; two required explantation due to persistent troublesome floaters.
Conclusions:
Pinhole sulcus intraocular lens can be implanted in patients who are symptomatic with glare or poor vision as a result of corneal astigmatism caused by penetrating keratoplasty, lamellar keratoplasty or scarring; or in patients with iris trauma. Visual outcomes were variable in our study group with high complication rate in some.
