Changes in corneal density after allograft corneal inlay implantation for hyperopic patients

Purpose:
To determine and compare the changes in corneal density (CD) after allograft corneal inlay (ACI) implantation in patients with hyperopia.
Setting:
Istanbul Medipol University, Faculty of Medicine, Ophthalmology Department, Istanbul, Turkey.
Methods:
After femtosecond flap creation, the excimer laser shaped ACI with attempted refractive power change determined according to patients' refractive error was placed in the center of the pupil and implanted in all patients. Anterior corneal density (ACD, 120 µm) and total corneal density (TCD) was evaluated with a rotating Scheimpflug system at baseline and 1, 3, 6 and 12 months after ACI implantation in eyes. ACD and TCD were evaluated in the 3 different zones (Zone 1; concentric radial zones 0–2 mm and Zone 2; concentric radial zones 2–6 mm; Zone 3: whole cornea) located around the apex.
Results:
Thirty-one eyes with a refractive error between 1.00 and 6.00 D of 17 subjects were included in this prospective study. The mean age of the patients included in the study was 38.5±12.7 (range 22-65) years. Twelve months after surgery, the mean optical ACD values in zone 1, zone 2 and zone 3 increased significantly from 18.6±2.6, 17.5±2.0, 20.4±3.5 to 20.6±1.6 (p=0.002), 18.2±1.2 (p=0.003), 21.4±3.1 (p=0.009) respectively. The mean optical TCD values in zone 1, zone 2 and zone 3 increased significantly from 14.7±1.4, 13.5±1.1, 16.6±3.0 to 15.7±1.0 (p=0.002), 14.1±0.8 (p=0.004), 17.1±2.7 (p=0.026) respectively.
Conclusions:
Allograft corneal inlay implantation is a new treatment method in the optical correction of hyperopic patients. In this group of patients, the ACD and TCD values were found to almost returned to the pre-operative baseline level at the 12 months follow-up. The measured values were significantly different, however, considered not to be clinic relevant.
