Multi-component assessment of perceived visual quality with presbyopic corrections using MAS-2EV

Abstract Details

Purpose:

To evaluate visual quality with several presbyopic corrections (PC) in different conditions (day, night, near, far and stereo) and their visual compromise, using the Multifocal Acceptance Score (MAS-2EV) metric, and a Binocular Simultaneous Vision Simulator (SimVIsGekko)

Setting:

Methods:

MAS-2EV is a platform of images (daytime & nighttime scenes) at far (monitor @4m), and near (iPad). The multicomponent metric encompassed perceptual scores (PS) of the images (0-10), and near stereo-acuity. Thirty-eight patients (age:72±9yrs) participated in the study: 5 presbyopes (accommodation amplitude:1.35±0.33D), 25 with cataracts (OSI:1.1-7.6), and 8 pseudophakic (monofocal IOLs). Patients judged the images (PS) through corrections simulated by the SimVisGekko (2EyesVision): binocular bifocal lenses (BB); Monovision (FN); Modified monovision (FB); and binocular far vision (FF). Visual Compromise was estimated from Visual Degradation @far (PS @F_PC–PS@F_FF)/PS@F_FF, and Visual Benefit @near (PS@N_PC PS@N_FF)/PS@F_FF. 

Results:

MAS2-EV scores for presbyopic/pseudophakic patients @far were 9.5±0.7/9.3±0.9 (FF), 7.5±0.7/7.9±1 (FN), FB: 8±0.6/8±0.8 (FB), 4.6±0.7/4.1±1.8 (BB); 0.67/1.1 mean STD across patients-corrections; and @near 4.8±2.4/6.9±1.3 (FF), 7.8±1.6/7.8±2.5 (FN), 5.31±2.5/6±1.9 (FB), 5.3±2/5.4±3.6 (BB); 2/2.3 mean STD across patients-corrections. The visual benefit @near stereopsis was FN:24±35.7%, FB:12±30.3%, BB:83.9±21.9%. The visual benefit @near was FN: 32±20.7%/8.2±21.5%/28.3±32.3%, FB: 5.6±13%/-10.3±12.8%/1.7±17.5% & BB: 5.3±21.3%/-17.2±41%/3.6±46.5%, and the visual degradation @far was FN: -21.2±2.7%/-15.2±10.8%/-23.2±9.2%, FB: -15.3±3.1/-14.8±6%/-23.2±9.2%; & BB: -51.5±7.6%/-55.6±20.5%/-74.4±21.5%, for presbyopic/pseudophakic/cataract patients with OSI<2.

Conclusions:

MAS-2EV in combination with SimVis Gekko simulating presbyopic corrections allows a comprehensive and rapid assessment of vision before implantation. The high intersubject variability in the visual compromise of different corrections suggest that pre-operative evaluation with SimVis Gekko/MAS-2EV will guide the optimal correction.
