Optical performance of a capsular bag trifocal IOL in comparison with a supplementary trifocal IOL duet combination

Purpose:
Trifocality can be achieved with a polypseudophakic approach: a monofocal/toric IOL is implanted in the capsular bag, in combination with a trifocal supplementary lens in the sulcus. While reversibility could be an advantage, one may question if the supplementary lens duet approach offers comparable image quality to a single-lens trifocal lens.

Methods:
In this study, we compared in vitro the optical performance of a two-lens combination; a monofocal RayOne Aspheric (RAO600C, Rayner) plus a Sulcoflex Trifocal (IOL703F, Rayner) against a single-lens RayOne Trifocal (RA0603F, Rayner) and competitor trifocal IOLs (PanOptix [Alcon], FineVision Pod F [PhysIOL], AT Lisa Tri [Carl Zeiss]).
The Modulation Transfer Function (MTF), an internationally recognized standard for testing IOLs’ optical quality, and the Strehl ratio were assessed with the optical bench OptiSpheric IOL PRO2 (Trioptics).
Results:
The polypseudophakic approach and all trifocal IOLs showed comparable performance for a photopic pupil. The highest and the lowest Strehl value differed only by 0.04, 0.05 and 0.07 at the far, intermediate and near focus, respectively. The MTF curves of the two-lens combination and the single trifocal IOLs were comparable. The USAF-chart photographs confirm comparable image quality of the IOLs at 3 mm: differences between the monofocal RayOne plus the Sulcoflex Trifocal, and the trifocal lenses for capsular bag implantation were hardly noticeable.
Conclusions:
A polypseudophakic combination of the Sulcoflex Trifocal and the monofocal RayOne Aspheric provides good MTF performance at the distant, intermediate and near focal points, comparable to that of the RayOne and competitor trifocal lenses for capsular bag implantation. Optical quality is not affected when using a polypseudophakic approach.
