Comparative analysis of visual results and subjective satisfaction of patients after implantation of two models of trifocal diffractive intraocular lenses

Abstract Details

Purpose:

To evaluate and compare the visual outcomes after implantation of two models of diffractive trifocal IOLs using decimal scale, LogMAR system, interactive computer program (ICP), contrast sensitivity function (CSF) and subjective assessment of visual quality

Setting:

Methods:

Patients with binocularly implanted trifocal IOLs of two different models AcrySof IQ PanOptix (TFN) (34 eyes) and AT LISA tri 839 MP (TRI) (34 eyes) were evaluated at 1 year after surgery. Postoperative examination including uncorrected visual acuity (UCVA) and best corrected visual acuity (BCVA) in 4 m, 80 cm, 60 cm and 40 cm using decimal scale, LogMAR system and ICP; CSF under photopic and mesopic conditions (Optec)and a questionnaire on subjective quality of vision (VF-14, Steinberg EP, 1994), optical phenomena and spectacle independence was performed. ICP included the modified three-bar stimuli in two orientations, tumbling-E in four orientations

Results:

UCVA at distance (0,88 vs 0,77, p=0,03), near (0,89 vs 0,76, p=0,06) and intermediate distance (0,93 vs 0,79, p=0,03) was better in TFN group in comparison with TRI measured with decimal scale, ICP and LogMAR system. Under photopic and mesopic conditions with and without glare at all spatial frequencies the CSF was higher after implantation of TFN in comparison with TRI, p=0,02. CSF had typical configuration with maximum figures in the area of intermediate spatial frequencies (SF) and reduction in the field of high SF for the both trifocal IOLs groups, however better indicators were registered in TFN group

Conclusions:

High visual outcomes at all distances were observed in both groups with better results in TFN group on VA, CSF and subjective satisfaction of visual quality. Evaluation of VA-results using various systems showed the advantage of the ICP method. Mean spherical equivalent was 0,11±0,61 D in TFN group and -0,38±0,72 D in TRI group (p=0,009). The average VF-14 score was 97.4 and 96.7 for TFN and TRI groups respectively (p=0,61), showeing high level of functional results and patients satisfaction. The percentage of spectacle correction after surgery was 4.6 % less in TFN group (13,6 %) compared to TRI (18,2 %)
