Comparative evaluation of visual outcomes after bilateral implantation of an advanced monofocal intraocular lens and a conventional monofocal intraocular lens

Abstract Details

Purpose:

To compare clinical outcomes and patient satisfaction of Tecnis Eyhance, an advanced monofocal intraocular lens (IOL) with a conventional monofocal IOL.

Setting:

Retrospective case-control study Garcia de Orta Hospital, Almada, Portugal

Methods:

Our study included 120 eyes of 60 patients (30 patients in each group) who had indication for bilateral cataract surgery, above the age of 65 years. These patients underwent bilateral cataract surgery with implantation of a conventional monofocal IOL (Tecnis PCB00) or an advanced monofocal IOL (TECNIS Eyhance ICB00) in both eyes. Ophthalmological evaluation included measurement of monocular and binocular uncorrected distance visual acuity (UDVA) and corrected distance VA (CDVA) and uncorrected intermediate VA (UIVA). Additionally, binocular defocus curve was analyzed, vision and life quality were also assessed through a subjective questionnaire.

Results:

The UDVA was 20/22 in the ICB00 group and 20/20 in the PCB00 model and the CDVA was 20/20 in both groups (p> 0.05). Monocular and binocular UIVA were statistically significant better for the ICB00 group than for PCB00 group. In monocular, the UIVA was 20/32 in the ICB00 group and 20/40 in the control group (p <0.05). In binocular, there was a mean value of 20/30 in the ICB00, compared to 20/40 in the control group (p <0.05). The resulting defocus curves showed that the ICB00 model presented a longer plateau with sustained visual acuity greater than 20/40 through more diopters when compared to PCB00 (range +1.00 to -1.50 D). The reported dysphotopsia in both groups was not significantly different between both groups (p=0.56). Regarding the quality of life questionnaire, the ICB00 group demonstrated less difficulty in activities requiring intermediate vision.

Conclusions:

The results of this study demonstrated a significant improvement in visual acuity for intermediate distance in the ICB00 group compared to the control group, without compromising distance visual acuity. There was also greater independence of glasses in activities of intermediate distance (namely for computer use) with the ICB00 model. Nowadays, there is greater demand and expectation from patients for intermediate activities. In this context. the ICB00 IOL stands out for its innovative design and might revolutionize the group of monofocal IOLs.
