Purpose:
The aim of the current study is to report and compare the visual and refractive outcomes with
the AkkoLens accommodative intraocular lens (AIOL) after cataract or refractive lens exchange
surgeries in eyes with and without corneal refractive surgery prior lens AIOL implantation.
Setting:
1. Division of Ophthalmology, Miguel Hernandez University, Alicante, Spain.
2. Miranza - VISSUM INNOVATION, Alicante, Spain.
3. Akkolens International, The Netherlands.
4. Ocular Diopter Node, OFTARED - RETICS, Spain.
Methods:
This study included 20 eyes of 10 patients that were implanted with the AkkoLens AIOL. Patients were divided in two groups; group A, 14 eyes of 7 patients that had untreated cornea prior AkkoLens AIOL implantation, and group B, 6 eyes of 3 patients that had corneal refractive surgery prior AIOL implantation. Distance and near visual acuities and refraction were measured. The postoperative follow-up period was from 1 to 6 months. For the statistical analysis of the results, non-parametric tests (U-Mann Whitney) were applied in the SPSS Statistical program.
Results:
The results showed statistically significant differences in the postoperative addition (p:0.041), being statistically higher in eyes underwent prior corneal refractive surgery than in untreated eyes before AIOL implantation (1.92±0.13D and 1.69±0.24D respectively) No statistical significant differences between groups were detected in the rest of visual and refractive outcomes (p≥0.05). At the preoperative level, statistically significant differences were obtained in uncorrected distance visual acuity, corrected distance visual acuity, cylinder and addition (p:0.006, 0.012, 0.005 and 0.012, respectively), obtaining better outcomes preoperatively in patients with refractive surgery prior to the implantation of the AkkoLens lens.
Conclusions:
Visual and refractive state provided by AkkoLens AIOL in patients who underwent prior corneal refractive surgery was comparable to patients without prior refractive surgery, except for addition values, which were slightly higher in patients with prior refractive surgery. Preoperative refractive state was better in patients with corneal refractive surgery, which is consistent with the objectives of refractive surgery.. The implantation of the AkkoLens improves near vision in eyes that underwent prior corneal refractive surgery and in untreated eyes(1.92±0.13D and 1.69±0.24D respectively)
