Short-term clinical outcomes after unilateral refractive lens exchange (RLE)

Abstract Details

Purpose:

To report on the clinical outcomes of emmetropic presbyopic patients following unilateral RLE with an advanced technology intraocular lens (IOL).

Setting:

Optegra Eye Hospitals, UK

Methods:

A retrospective audit of clinical outcomes of consecutive patients following unilateral RLE surgery with multifocal or extended depth of focus (EDOF) IOL implantation. Inclusion criteria were RLE patients with fellow eyes with an uncorrected distance visual acuity (UDVA) of 20/30 or better and spherical equivalent refraction (SEQ) between -1.00D to +1.00D. Data audited were recorded in an electronic medical record typically at 3 months post-operatively.

Results:

Preliminary data from 89 eyes of patients (mean age 58±7 years) and a pre-operative SEQ of +0.73±1.74D were included. All surgeries were uncomplicated and 93% of eyes had no postoperative complications. 86% of eyes achieved a postoperative UDVA of 20/30 or better with 95% of eyes achieving a postoperative mean SEQ within ±1.00D of predicted target. 85% of patients achieved binocular UDVA of 20/20 or better and 94% could read N8 or better unaided. There were no documented cases of surgery being undertaken for the fellow eye, which was taken as a possible indicator of treatment success.

Conclusions:

Unilateral RLE with multifocal or EDOF IOL implantation is a safe and efficient procedure for emmetropic presbyopes. Our results demonstrated a significant improvement in unaided near vision post-operatively, without detriment to distance vision. This approach has potential for reduced dysphotopsia and night vision symptoms (subjective feedback collection is ongoing), whilst reducing surgical risk as compared to more traditional bilateral RLE surgery.
