Posterior capsule dynamics and visual quality after implantation of hydrophobic acrylic IOL with UV/Ozone posterior surface modification

Abstract Details

Purpose:

To assess the posterior capsule (PC) dynamics after implantation of a posterior surface modified (ultraviolet/ ozone treatment) intraocular lens (IOL) and correlate the PC to IOL distance with visual quality.

Setting:

Dr Rajendra Prasad Centre for Ophthalmic Sciences, AIIMS, New Delhi, India.

Methods:

Prospective interventional study evaluating the PC dynamics in 42 eyes implanted with hydrophobic acrylic IOL (posterior surface ultraviolet/ ozone treatment) after phacoemulsification. The primary outcome measure was PC configuration (wavy/smooth) and adhesion to the IOL optic on anterior segment optical coherence tomography (ASOCT). Secondary outcome measure was visual quality, including higher order aberrations (HOAs), modulation transfer function (MTF) and Strehl Ratio as assessed on ray tracing aberrometry. Follow up was performed on POD 1 and 1 year.

Results:

On POD 1, PC was wavy in 40.5% (17/42) and smooth in 59.5% (25/42) of cases. No case had complete PC-optic adhesion. At 1-year, complete PC-optic adhesion was seen in 66.7% (28/42) of cases. Complete adhesion was significantly higher in cases with initial smooth PC (88.2% smooth, 52% wavy; p=0.02). Strehl ratio (p=0.011) and MTF (p=0.027) were significantly better with complete PC-optic adhesion. Residual PC-optic distance (Range 0-182 µm) positively correlated with total HOA (Pearson coefficient=0.365, p=0.017). No association was observed between PC-optic distance and posterior capsular opacification (p>0.05).

Conclusions:

PC configuration on POD1 impacts final visual quality and adhesion to IOL optic.
