Comparison of ICL vault measurements between pentacam sheimpflug tomography and anterior segment optical coherence tomography

Abstract Details

Purpose:

To compare interobserver and interdevice reproducibility of Implantable Collamer Lens (ICL) vault measured by Pentacam Sheimpflug tomography and anterior segment optical coherence tomography (AS-OCT).

Setting:

Ophthalmology Department, Centro Hospitalar Universitário do Porto

Methods:

Cross-sectional study of patients submitted to phakic ICL (V4c) implantation for myopic refractive error correction, with a minimum follow-up of 12 months. Twenty-one eyes of 11 patients were included. All eyes underwent imaging evaluation with Sheimpflug tomography (Pentacam® HR) and AS-OCT (SPECTRALIS® Heidelberg Engineering). Both devices were used to measure the central ICL vault by 3 observers. In the statistical analysis, the interobserver and interdevice measurements agreement were accessed by calculating intraclass correlation coefficient (ICC). An ICC greater than 0.80 indicated very strong agreement. All data were arranged, processed, and analyzed with SPSS v.24.0 data.

Results:

For observer 1-3, median central ICL vault was 92 µm (range 50-740), 140 µm (range 60-790) and 140 µm (range 65-765) measured by Pentacam, and 160 µm (range 34-958), 140 µm (range 38-967) and 153 µm (range 43-961) measured by AS-OCT, respectively. In the interobserver analysis, the ICC was 0.94 (95% CI 0.81-0.98) with Pentacam and 0.99 (95% CI 0.99-1.0) with AS-OCT. In the interdevice analysis, the values were comparable according to ICC (0.92, 95% CI 0.87-0.95). In addition, the median central ICL vault measured by Pentacam was significantly lower (130 vs 153 µm, p=0.001).

Conclusions:

There was very strong interobserver and interdevice reproducibility of ICL vault measured by Pentacam and AS-OCT. Both devices showed comparable values of ICL vault, although it seems to be a trend towards lower values with Pentacam. The use of AS-OCT should be considered whenever possible, since this tool showed higher interobserver reproducibility, probably related to better visualization of the intraocular lens and crystalline boundaries.
