Comparative evaluation of visual outcomes in small incision lenticule extraction vs implantable collamer lens in moderate myopia

Abstract Details

Purpose:

To compare postoperative visual outcomes in patients undergoing Small Incision Lenticule Extraction (SMILE) and Implantable Collamer Lens (ICL) implantation for correction of moderate myopia.

Setting:

Rajendra Prasad Centre for Ophthalmic Sciences, All India Institute of Medical Sciences, New Delhi

Methods:

A prospective comparative study was performed in cases of moderate myopia in which 40 eyes that underwent SMILE were compared with 40 eyes that underwent ICL procedure. At day 1, 1 month and 3 months post-operatively, the parameters evaluated were uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), intraocular pressure (Non-contact Tonometry), corneal topography (Pentacam), ocular aberrations (iTrace), contrast sensitivity (Pelli Robson Chart) and endothelial cell density (Specular microscopy).

Results:

The baseline parameters of both the groups were comparable. The visual acuity improvement was comparable in both the groups. Total higher order aberrations were increased in SMILE group (0.20±0.08; 1 month and 0.17±0.06; 3 months) but remained almost unchanged as preoperative in ICL group (0.12±0.04; 1 month and 0.12±0.03; 3 months) and there was statistically significant difference between the 2 groups (p<0.001). Postoperative spherical aberration and coma at 3 months was significantly more in SMILE group (p<0.0001). The contrast sensitivity on Pelli-Robson chart was significantly better in ICL group (p<0.001). No significant difference existed in final residual refractive error. Endothelial cell loss was not significant in either group at 3 months.

Conclusions:

ICL provides better quality of vision than SMILE in cases of moderate myopia although the quantitative visual improvement is comparable.
