Posterior chamber phakic intraocular lens (PCPIOL) for patients with keratoconus

Abstract Details

Purpose:

To show our experience in the treatment of ametropias with a posterior phakic intraocular lens for patients with keratoconus and the results according to their clinical characteristics.

Setting:

Corneo-plastic unit, Queen Victoria Hospital. East Grinstead. The UK.

Methods:

Retrospective review of patients with stable keratoconus treated with toric and non-toric PCPIOL (ICL Staar Surgical and IPCL Care Group) or to improve their visual acuity. Data were obtained from pre-operative visit and 1, 3, 6 and 12 months after surgery. Clinical characteristics, pre and post uncorrected (UCVA) and best-corrected (BCVA) logMAR visual acuities, perioperative complications were analyzed.

Results:

23 eyes of 21 patients were included. UCVA changed from 0.75 ± 0.3 preoperatively to 0.2 ± 0.22 post-surgery and BCVA from 0.07 ± 0.2 to 0.13 ± 0.15. 70% of the cases increased 3 or more Snellen lines of UCVA while none of the eyes lost more than 2 Snellen lines of BCVA. No significant difference in final UCVA was founded between patients with or without previous keratoplasty (0.27 and 0.18, P=0.38), previous corneal collagen crosslinking (0.16 and 0.3, P=0.24) or intracorneal ring segments (0.2 and 0,2, P=0.94). 4 rotations (17%) after surgery and 1 inverted insertion were observed.

Conclusions:

The use of PCPIOL in patients with keratoconus is effective in improving their UCVA, even in cases with previous corneal procedures such as keratoplasty, crosslinking and intracorneal rings. Rotation is the most common postoperative complication
