Purpose:
To evaluate the efficacy and safety of implanting a phakic collamer posterior chamber lens (ICL) for visual rehabilitation after penetrating keratoplasty
Setting:
Centro de Oftalmología Barraquer. Barcelona, Spain.
Methods:
This retrospective, longitudinal study included a single cohort of patients who underwent myopic or toric ICL implantation after penetrating keratoplasty (PK). Patients included met the following inclusion criteria: prior PK at least 12 months before ICL implantation, minimum follow-up of 2 months, transparent graft, absence of sutures and ACD >2.8 mm depth. Patients with unstable refraction or other procedures performed between PK and ICL implantation were excluded. Standard ICL implantation technique was performed. Toric ICL were aligned using a digital intraoperative system. Main outcome measures were UCDVA, BCDVA, post-op sphere and cylinder, and efficacy and safety indexes.
Results:
Thirty-one eyes of 26 patients were included. Mean age was 35±8(20/54) years. Time between PK and ICL implantation was 94±64(15/246) months. Mean follow-up was 22±17 months. Pre-op ACD was 3.69±0.44 mm and pre-op endothelial cell density was 1355 ±796 cells/mm2. Pre-op sphere was -6.68±5.96 diopters(D) and cylinder 4.81±2,46 D. Pre-op Snellen decimal UCDVA and BCDVA were 0.05±4.42 and 0.55±1.54 respectively.Post-op sphere was -0.31±0.60 D and post-op cylinder was 0.51±0.82 D. Post-op UCDVA and BCDVA were 0.46±2.29 and 0.73±1.21 respectively. Efficacy index was 0.85 and safety index was 1.32. No complications were observed during the follow-up period.
Conclusions:
Phakic toric or spheric collamer posterior chamber lens implantation is a safe and effective surgical procedure to rehabilitate post-penetrating keratoplasty eyes in which glasses, contact lenses or cornea-based refractive procedures are not indicated.
