Optomap ultrawide field imaging for detecting peripheral retinal lesions in 1725 high myopic eyes before implantable collamer lens surgery

Abstract Details

Purpose:

To evaluate specificity and sensitivity of nonmydriatic Optomap ultrawide field imaging for detecting peripheral retinal lesions in high myopia patients before implantable collamer lens (ICL) surgery.

Setting:

Identifying peripheral retinal lesions in high myopia patients before implantable collamer lens (ICL) surgery by nonmydriatic Optomap ultrawide field imaging.

Methods:

A consecutive, prospective, observational study in 1725 high myopic eyes of 897 ICL surgery candidates. Patients with high myopia were scheduled for routine ophthalmic examination before ICL implantation. Nonmydriatic Optomap ultrawide field image were applied for potential peripheral retinal lesions before ICL surgery. Generalized Estimating Equations (GEE) were applied to analyze risk factors of high myopia related peripheral retinal lesions.

Results:

344(19.94%) of 1725 eyes had peripheral retinal lesions in total. 75(4.35%) eyes had holes/tears. 313(18.14%) had peripheral retinal degenerations, of which 164(9.44%) had lattice degeneration, 145(8.35%) had pigmentary retinal degeneration. Optomap detected peripheral retinal holes/tears and degenerations with sensitivity of 57.33%(95%CI:45.38%-68.69%), 65.18%(95%CI:59.61%-70.45%) and specificity of 99.58%(95%CI:99.13%-99.83%) and 99.08%(95%CI:98.43%-99.51%). Kappa value of assessing peripheral retinal holes/tears, degenerations between Optomap and Goldmann three-mirror contact lens examination was 97.74%(κ=0.68±0.05) and 92.93%(κ=0.73±0.02). Longer axial length (OR:1.16, P<0.01), more severe refractive error (OR:1.05, P<0.01) and older age (OR: 1.03, P<0.01) were proved to be risk factors for peripheral retinal degeneration in high myopes.

Conclusions:

Nonmydriatic Optomap ultrawide field imaging is a reliable adjunctive tool with high specificity and moderate sensitivity for detecting and recording peripheral retinal lesions in high myopes before ICL surgery.
