Cross-linking treatment for low-grade myopia:12-months results for comparison of two ring-shaped treatment protocols

Purpose:
We here compare the efficacy and safety of two different ring-shaped crosslinking protocols for low-grade myopia.
Setting:
Department of Clinical Sciences/Ophthalmology, Umeå University, Umeå, Sweden
Methods:
In an single-masked, intra-individually comparing study 27 patients with bilateral low-grade myopia (spherical equivalent , SE -0.75 - -2.50) were treated with bilateral Photorefractive Intrastromal CrossLinking (PiXL); one eye with a 4-mm ring shaped protocol, the other with a 3.5-mm ring shaped protocol, which was randomized. Uncorrected (UDVA) and corrected visual acuities (CDVA), low-contrast visual acuities, refractive spherical equivalents (SE), endothelial cell count (ECC) and adverse events were analyzed pre- and post treatment at 1, 3, 6 and 12 months.
Results:
In both protocols, improved UDVA and SE were seen through 12 months, better with the 3.5-mm protocol (-0.50±0.20 Log MAR vs. -0.39±0.18 LogMAR; p=0.007, and 1.14±0.44D vs. 0.95±0.38D; p=0.038, respectively). A decline in low-contrast visual acuities that persisted though 12 monts was seen with the 3.5-mm protocol, but not with the 4.0-mm protocol. The ECC and CDVA remained unchanged. No adverse events occurred.
Conclusions:
Our results indicate that the 3.5-mm protocol gives a larger reduction in myopia with a corresponding larger improvement in UCVA, but possibly to the expense of decreased low-contrast visual aculities. Possible consequences will be discussed.
