Abstract Details

Purpose:

To evaluate the efficacy and safety of micropulse transcleral cyclophotocoagulation and glaucoma drainage devices (Ahmed valve) for elevated intraocular pressure after penetrating keratoplasty.

Setting:

study done in Magrabi Eye &Ear Hospital, Jeddah, Saudi Arabia.

Methods:

Retrospectively evaluating transcleral cyclophotocoagulation was done for 14 eyes represent the 1 group and the 2nd group Ahmed’s valve had been implanted for 14 eyes status post penetrating keratoplasty not controlled on maximal tolerable medical anti glaucoma therapy,. demographic information ,glaucoma type, surgical details ,preoperative and post-operative data including intraocular pressure (day 1,1week,1 month and every 3 months ),number of mediation , post-operative complications and any follow up procedure recorded. Complete success considered when IOP < 21mmhg without medication , qualified success means that additional medication needed and failure procedure when either need for repeated surgery for glaucoma.

Results:

average age of the study sample was 37.5Yrange from 9-64 year.average preoperative IOP was 34 mmhg range from (22-48), post operatively was 13.86 In 1st week, 15.6 after 1 month , 17.1 after 3 months, after 6 months was 21.4 and 18.2 after 1 year follow up for the 1st group and post operative IOP was 15,15.7,17.3,18.3,16.5 on 1 week ,1 month , 3 months , 6 months and 1 year respectively for the 2nd group. Average number of anti-glaucomatous drug usage deceased from 2.99 preoperatively to 1.2 for the 1 st group and 0.5 for the 2 nd group 1 year postoperatively.

Conclusions:

Post operative IOP control show no statistical difference between the two groups , however transcleral cyclophotocoagulation group shows less complication. randomized, large prospective studie was advised to determine the best surgical approach for the mangagmnet of elevated iop in eyes that have undergone PK.
