Towards on the safety and effectiveness of suprathreshold selective laser trabeculoplasty

Purpose:
The aim of the work was to study the safety and effectiveness of this technique.

Methods:
We studied 48 cases of exceeding SLT, where the laser energy was set at which the effect of "champagne spray" was achieved, and then increased by 0.1 mJ and SLT was performed at the selected values. In group 1 (1st degree of pigmentation of the anterior chamber angle (ACA)) preoperative Р0 – 16.52 ± 2,6 mmHg, С=0,12 ± 0,03* mm3/mm Hg*min, visometry – 0.3±0.29, endothelial cell density (ECD) – 2243±366 cells/mm2. In group 2 preoperative Р0 - 16,2±2,4 mmHg, С=0,11 ± 0,03* mm3/mmHg*min, visometry - 0.3±0.28, endothelial cell density (ECD) - 2250±348 cells/mm2. Photoregistration of the ECD has been completed.
Results:
Colorimetric analysis showed a decrease in pigmentation of the trabecular network. Postoperative tonography parameters were Р0=14.3 ± 3.80 mmHg, С=0,16± 0,03 mm3/ mm Hg * min, visometry - 0.3±0.28, ECD - 2204 ± 392 cells / mm2. In group 2 Р0 - 14,4±1,8 mmHg, С=0,16 ± 0,03* mm3/ mm Hg * min, visometry - 0.3±0.27, endothelial cell density (ECD) - 2198±290 cells / mm2. Hypotensive effect was achieved, reactive hypertension was not observed. In no case was noted ciliary injection, pain on palpation in the projection of the ciliary body, precipitates, or Tyndall phenomenon.
Conclusions:
According to the study, SLT performed at sup-threshold energy values is an effective and safe method of laser treatment of primary open-angle glaucoma.
