SMILE with low energy level: assessment of dissection quality and optical quality recovery results with three laser modules

Purpose:
To compare the results of Small-Incision Lenticule Extraction (SMILE) with a low laser pulse energy (100 to 105 nJ) for myopia and myopic astigmatism treatment, obtained with three different laser modules.

Donate Vision Clinic, France
Methods:
This retrospective study included 179 eyes of 179 consecutive patients separated into 3 groups, all treated with the same VisuMax device, but each with a different laser module integrated. The pattern bubble and lenticule dissection were graded according to 5 grades (1 to 5): very severe to clear and very difficult to very easy, respectively. Visual acuity (VA), Contrast Sensitivity (CS), Refraction, Corneal High-order Aberrations (HOAs), Modulation Transfer Function Cut off (MTF cut off), Objective Scatter Index (OSI) were measured in preoperative, at 1 day and 1 month postoperatively.
Results:
In three groups, no significative difference in the pattern bubble was observed and all lenticule were removed without difficulty. At 1 month, all eyes recovered 20/20 or better of CDVA and the preoperative CS values at 1 month in three groups with no significative difference in safety index (1.15, 1.14 and 1.12, respectively in group 1, 2 and 3). At 1 month, no significative difference was found in mean SE with 0.3 ± 0.24 D, 0.17 ± 0.23 D et 0.20 ± 0.29 D respectively in group 1, 2 and 3. The efficacy index showed no significative difference with 1.04, 0.98 and 1,02, respectively in group 1, 2 and 3 at 1 month. No difference in the optical quality measures (Corneal HOAs, MTF, SR and OSI) were found between each group at 1 month.
Conclusions:
This study proved the use of laser energy near to the plasma threshold was effective and safe with 3 different laser modules, without any differences considering the dissection quality and the optical quality recovery.
