Late onset, persistent, recurrent low grade post-operative endophthalmitis caused by Staphylococcus arlettae cultured from viscoelastics: a study of non-clustered 4 cases

Purpose:
Post-cataract surgery late onset low grade vitritis has several confounding causes, tricky amongst them being culture negative endophthalmitis, where only a high index of suspicion made us culture the viscoelastic syringes.
Setting:
Four uneventful cataract surgery cases operated on different dates in December 2019 and after reaching post-operative 6/6 visual acuity showed low grade vitritis 3-4 weeks later with no significant anterior chamber inflammation.
Methods:
All 4 cases showed a negative vitreous smear and culture. All 4 cases were injected with vancomycin with piperacillin and tazobactam after a 48-hour observation period of no response to topical with/without oral steroids and antibiotics.
Results:
One recovered uneventfully to 6/6 vision, the other three required vitrectomy, of which one resolved to 6/6 vision, the second resolved to 6/6 vision but developed effusive choroidals that required drainage with silicone oil injection and the third deteriorated very rapidly to giant retinal tear with hemorrhagic choroidals with a no PL outcome.
Conclusions:
Staphylococcus arlettae isolated only from viscoelastic samples is a rare organism that causes late onset, usually culture negative, post-operative endophthalmitis with a very varied clinical spectrum.
