High order aberrations in four corneal transplantation techniques

Abstract Details

Purpose:

To describe and compare the corneal higher-order aberrations of the anterior, posterior corneal surfaces and total cornea in eyes that underwent Descemet`s membrane endothelial keratoplasty (DMEK), Descemet Stripping with Automated Endothelial Keratoplasty (DSAEK), deep anterior lamellar keratoplasty (DALK) and penetrating keratoplasty (PK).

La Mancha Centro Hospital

Methods:

A retrospective comparison of consecutive eyes which underwent: DMEK 28 eyes, DSAEK 37 eyes, 32 eyes which underwent DALK, and 26 PK, and 40 uncomplicated eyes of 40 control patients. The keratoplasties were realized by a single surgeon. In postoperative annual examination, each patient was analyzed with Pentacam (Pentacam HR, Oculus, Wetzlar Germany). The high order aberrations were calculated using the Pentacam software for Zernike analysis based in height. The central 4 and 6 mm area up to sixth Zernike order were analyzed. Outcome data were compared between groups.

Results:

We found significant differences in higher order aberrations of the anterior, posterior surface and total cornea in the central 4 mm area of the four corneal transplant surgery techniques. These differences were higher in the central 6 mm. Control eyes had significantly lower total high order aberrations of the anterior and posterior surface, and total cornea than the other groups.

Conclusions:

Our study showed that keratoplasty techniques induced high order aberrations, which were different according to layer or total cornea transplant. Evaluation of the corneal high order aberrations is helpful to understand the quality vision in the several keratoplasty techniques.
