Outcomes of a 320 degree intrastromal corneal ring segment implantation in special cases for keratoconus

Abstract Details
Purpose:
To evaluate the visual acuity, refraction, corneal tomography and corneal asphericity after Ferrara 320-degree intrastromal corneal ring segment (ICRS) implantation in advanced stage of the keratoconus.
Setting:
Davinci Eye Clinic, Tbilisi, Georgia.
Methods:
The study included 16 eyes having K Max from 60.2 D to 76.1 D preoperatively (mean 64 D). Surgeries were performed using a manual method for tunnel creation followed by 320-degree ICRS implantation. The preoperative and postoperative uncorrected (UDVA) and corrected (CDVA) distance visual acuities, spherical equivalent (SE), corneal and refractive astigmatism, keratometry (K) readings - K1, K2, Km, Kmax and mean asphericity (Q) value using tomography images were acquired. The mean follow-up period was 16 ± 3 months.
Results:
The postoperative mean UDVA and CDVA with a significant spherical equivalent and refractive astigmatism were statistically better (p<0.05) than the preoperative parameters in all cases. All corneal tomographic parameters (corneal astigmatisms, K readings, and Q value) improved significantly (p<0.05) between the preoperative and postoperative intervals. Finally, the mean vectorial corneal tomographic astigmatism significantly improved until the last follow up.
Conclusions:
This study suggests that implanting a newer long-arc length 320-degree ICRS is a safe and effective procedure for treating patients with advanced stages of keratoconus.
