Abstract Details

Purpose:

To report the long-term clinical outcomes after Bowman layer (BL) inlay transplantation in eyes treated for progressive Keratoconus (KC).

Setting:

Retrospective, interventional case series conducted at the Netherlands Institute for innovative ocular Surgery.

Methods:

35 eyes of 29 patients with progressive advanced KC underwent BL inlay transplantation. Best corrected spectacle and contact lens visual acuity (BSCVA, BCLVA), Scheimpflug-based corneal tomography measurements (such as simulated and maximum keratometry (Kmean and Kmax) and central corneal thickness (CCT), thinnest point thickness (TPT)) and complications were evaluated up to 8 years after surgery. Kaplan-Meier analysis was performed to analyze the estimated success rate of the procedure. Outcome parameters were also analyzed and compared for two subgroups (Group 1 with preoperative Kmax ">" 69D (n=26) and Group 2 with preoperative Kmax "<" 69D (n=9)).

Conclusions:

BL inlay transplantation is a potential treatment option in eyes with more advanced KC. At a follow up time of 8 years, BL inlay transplantation provides corneal stabilization, preservation of contact lens tolerance and potential postponement of deep anterior lamellar or penetrating keratoplasty for the majority of these cases.
