Totally opacified hydrophilic IOL's (tertiary cataract): a case series

Abstract Details
Purpose:
To report a case series of opacified hydrophilic intraocular lens (IOL) with evaluation of its impact on vision, cause of opacification and outcomes of IOL exchange.
Setting:
Ophthalmology department, Sohag University Hospital, Sohag City, Egypt.
Methods:
A retrospective review of a non-comparative interventional case series including 12 eyes of 12 cases with opacified IOL which underwent IOL exchange by a hydophopic IOL. Preoperative data included the age, gender, systemic diseases, the time from cataract operation till presentation with opacified IOL, best corrected visual acuity (BCVA), anterior segment examination including the type of IOL and degree of its opacification, posterior segment examination. Operative data included the technique of IOL explantation, type of the new IOL and any reported complications. Postoperative data included the BCVA, refraction, IOL position and stability, examination of the explanted IOL.
Results:
The time of presentation was from 35 to 51(mean = 42.6) months after cataract operation. The mean logarithm of the minimum angle of resolution (LogMAR) visual acuity was significantly improved from 1.25 preoperatively to 0.32 at the final visit. Two patients had diabetes and the other 10 patients did not have any systemic disease. No signs of chronic uveitis were detected in any case. All explanted IOL's were foldable hydrophilic and when examined by operating microscope after being sectioned were found to be totally opacified at all thickness and not only the surface.
Conclusions:
Complete opacification of the hydrophilic IOL's is a newly recognized increasing complication of cataract surgery. The exact cause could not be detected, but the condition was not reported in hydophopic IOL's in this series.
