Intra-institutional retrospective analysis of IOL explantation in Japan during the last 14 years

Purpose:
To analyze cases which underwent IOL extraction, and IOL suturing or intrascleral IOL fixation, at one institute.
Setting:
Nishi Eye Hospital, Osaka, Japan
Methods:
We retrospectively investigated the cases of IOL extraction during the period from 2006 to 2020 at our institute, to figure out the IOL power and types, duration of implantation, and reasons for IOL explantation.
Results:
211 eyes in total underwent IOL extraction in 14 years. The main reason of IOL extraction was IOL dislocation (97eyes).
Among all, detailed data was available for 113 eyes, which underwent IOL extraction in median after 8.6years. IOL suturing was performed in 73 eyes. Intrascleral IOL fixation was performed in 40 eyes. In recent 2 years, intrascleral IOL fixation was performed in all cases, except few cases, in which the appropriate IOL power was not available and IOL suturing was essential. There was no severe postoperative complication such as bullous keratopathy or retinal detachment.
Conclusions:
IOL dislocation was the most common reason for IOL extraction. IOL extraxtion was performed safely in general.
Currently, in some cases, appropriate IOLs for intrascleral IOL fixation in patients with high myopia are not available. As intrascleral IOL fixation becomes more popular, the development of commercially available IOLs with more various ranges of IOL power is required, especially for patients with high myopia.
