Outcome of bag-in-the-lens (BIL) intraocular lens implantation in patients with uveitis

Abstract Details

Purpose:

To report the outcome in both pediatric and adult patients with uveitis who underwent cataract surgery and implantation of bag-in-the-lens intraocular lens (BIL-IOL).

Setting:

Cataract Department, Emergency Eye Clinical Hospital, Bucharest, Romania

Methods:

Patients with history of uveitis, who underwent cataract surgery (as a complication of uveitis) with BIL-IOL implantation between 2018 and 2020 were analyzed retrospectively. The following parameters were taken into consideration for both groups: 1) Cause of uveitis and age of diagnosis, 2) Severity and treatment of uveitis, 3) Complications of uveitis before surgery, 4) Impact of intraocular lens type and complications during surgery, 5) Postoperative outcome and long-term complications.

Results:

In total, 6 eyes of 3 children and 3 adults were included in the study: 3 unilateral cataracts, 2 bilateral cataracts and only one-eye cataract. For the pediatric eyes, median age at surgery was 12 years (range 10 years to 14 years). For the adult eyes, median age was 39 years (range from 36 to 42 years). The cause of uveitis was an an autoimmune disease in 2 adults and 2 children and in 2 cases cause remained unrevealed or not meeting the diagnostic criteria at the moment of surgery. Severity of ocular inflammation and younger age was related to the intraoperative difficulties. Postoperative complications rates, influencing significantly the visual outcome, were decreased in the adult group vs. pediatric group from 15% in adults to 48% in pediatric group. During the follow-up period, 2 eyes (33% in each group) developed glaucoma, 1 eye developed cystoid macular edema and no eye required treatment for visual axis opacification (VAO). No eye presented a dislocation of the BIL IOL or anterior capsular phimosis.

Conclusions:

The outcome of cataract surgery in uveitis patients is multifactorial due to the severity, the ocular complications and if the patient is under systemic treatment. Visual outcome had a strong correlation with the postoperative complications, but IOL implantation with the bag-in-the-lens gives good results with very low inflamation in both pediatric and adult cases after 1 year follow-up.
