Clinical and functional outcomes of Alcon Acrysof(SA60AT) and AMO(AR40e) lenses in paediatric cataract

Abstract Details

Purpose:

To evaluate clinical and functional outcomes of single piece SA60AT and three piece AR40e intraocular lenses(IOL) in paediatric cataract.

Setting:

Aravind Eye Hospital, Pondicherry

Methods:

Patients between 1-16 years age, who underwent surgery in 2015-16 were included. Refractive outcome, myopic shift, visual axis opacification(VAO), IOL centration, and postoperative complications were analyzed

Results:

165 eyes of 114 patients, with mean age of 7.79 years were included. Follow up ranged between 6 to 36 months. SA60AT was implanted in 131 eyes and AR40 in 34 eyes. Visual acuity(VA) improvement in 2-8 years(G2) and 8-16 years(G3) was significant (p<0.001), while in <2 years (G1) VA was less reliable. Median myopic shift of 0.5 and 0.06 was noted in G1 and G2, compared to G3(0). 8 eyes underwent Yag Capsulotomy while 2 needed membranectomy for VAO. 1 patient developed endophthalmitis.

Conclusions:

In developing countries the social and economic burden of health care in itself is a challenge. But the high incidence of preventable blindness such as cataract in the paediatric population, needs to be addressed with timely , standardized, appropriate, and cost effective approach. Incorporating premium IOL’s such as acrylic single piece or three piece lenses in the paediatric cataract management in a developing country may have its limitations due to the cost involved and patient’s affordability. Our experience with these IOL’s ( SA60AT and AR40e) were encouraging with respect to their functional characteristics , biocompatibility and cost effectiveness.
