MIOCT guided observations in posterior polar cataract surgeries

Abstract Details
Purpose:
To describe the plane of fluid currents during hydrodelineation and changes along the opacity-capsular junction during the entire period of phacoemulsification using microscope integrated optical coherence tomography (MIOCT).
Setting:
All India Institute of Medical Sciences
Methods:
A total of 12 (eyes) clinically confirmed posterior polar cataracts were operated under the direct guidance of Microscope Integrated Optical Coherence Tomography (MIOCT). The plane of separation during hydrodelineation and the changes along the opacity-caspular junction throughout the phacoemulsification procedure was noted. A stop and proceed technique was adopted to observe the capsular status and then to operate at every crucial step of phacoemulsification.
Results:
A successful delineation of plane of fluid currents during hydrodelineation and associated usual and/or inadvertent changes along the central opacity and the underlying capsule was noted successfully in all 12 eyes. In addition, the changes along the capsule following nucleus removal, following central opacity removal and following cortical clean-up was analyzed systematically in all cases. Inadvertent hydrodissection while performing hydrodelineation (n=1), fracture of the central opacity (n=1) and seepage of the fluid beneath the opacity (n=2) was observed in a total of 4 cases. Beneath the central opacity capsular opening was noted during policing in 1 case and continuous capsular Billowing was noted in two cases and amongst which one had capsular tear at the mid-periphery.
Conclusions:
MIOCT helps in understanding the crucial changes within the lens and more specifically along the capsular-opacity junction during the entire period of phacoemulsification in posterior polar cataract surgeries. Thus, broadening our understandings in these challenging situations to minimize avoidable complications.
