Surgical resection tactics of huge recurrent intracorneal and scleral cyst: a clinicopathologic report

Purpose:
To describe a novel surgical manner to treat huge recurrent intracorneal and scleral cyst.
Setting:
West China Hospital, Sichuan University, China
Report of Case:
A 9-year-old boy suffered from a recurrence of intracorneal and scleral cysts, which affected his pupil shape and refractive situation, underwent chemical injection and incision therapy for cyst approximately 6 years ago. After a careful evaluation of the characteristics of the cysts, we used an innovative surgical method to treat cyst a second time to reconstruct the ocular surface, restore vision, and improve appearance. There was no recurrence of the cyst after the second surgery in the 2 years follow-up. There were also no signs of corneal ectasia or fluctuations in vision. The corneal surface appeared smooth and healthy.
Conclusion/Take Home Message:
The surgery involved performing simple excision of the anterior wall of the cyst and the anterior corneal stroma only. We retained the posterior wall of the cyst as the anterior surface of the cornea to maintain a residual corneal thickness of more than 250 microns. This method minimized the risks of developing postoperative corneal ectasia and It was also a simple, economical, and effective treatment for giant corneal cysts.
