Purpose:
To report a case of microbial keratitis post photorefractive keratectomy (PRK)
Setting:
Department of Cornea and Refractive Services, Sadguru Netra Chikitsalya, Chitrakoot, Madhya Pradesh, India
Report of Case:
A 19 year old female underwent PRK for compound myopic astigmatism(CMA) in both eyes (BE).The preoperative refraction were(-5.00 diopter sphere {DS} /-1.00 diopter cylinder {DC} @180°[Right eye]{RE} and -5.50DS/-1.00DC @ 10° [Left eye]{LE}).No ocular or systemic co-morbidity was found in the patient. Post operative day 1, mild corneal infiltrates were seen in BE(RE >LE). Bandage contact lens (BCL) was removed and sent for culture. Meanwhile, eye drop(E/D) dexamethasone (DM) 6 times , artificial tears (AT) 6 times and moxifloxacin (MFC) 1 hourly were started in BE. On postoperative day 2, there was only mild haze in cornea and the patient was hence discharged on the same treatment. On followup day 8, the epithelial defect had healed and only mild corneal haze was present. One month later, the cornea had corneal haze and the refraction was( RE +3.00DS,LE +2.00DS/+0.50DC@20°)
Conclusion/Take Home Message:
With early identification of infectious organism and proper treatment, grave complications of post-refractive surgery keratitis can be prevented and reversed.
