Successful bioptics procedure for a minus 24 diopter myope: combined toric implantable collamer lens placement followed by small incision lenticule extraction

Purpose:
To report the safety and efficacy of a bioptics procedure for a -24 Diopter Myope combining Toric Implantable Collamer Lens (TICL) Placement followed by Small Incision Lenticule Extraction (SMILE) in Both Eyes (OU).
Setting:
Cleveland Clinic Abu Dhabi, Abu Dhabi, UAE
Report of Case:
A 21 year old female was referred to our refractive clinic for vision correction surgery. The manifest refraction of the right eye (OD) was -24.00 diopters (D)/+2.25D × 118 with an uncorrected distance visual acuity (UDVA) of count fingers (CF) at 3 feet and best spectacle corrected visual acuity (BSCVA) of 20/25. The manifest refraction of left eye (OS) was -24.75D/+2.75D × 88 with UDVA CF at 3 feet and BSCVA 20/25. Maximum power TICL’s were implanted OU. After TICL placement, the patient experienced improvement in refraction, UCVA and BSCVA OU. The manifest refraction OD was -6.75D/+0.50D × 47 with UCVA of 20/400 and BSCVA of 20/15. The manifest refraction OS was -8.25D/+0.75D × 63 with UDVA of 20/400 and BSCVA 20/15. After 2 years of monitoring, we then proceeded with SMILE OU to address residual myopic astigmatism. Postoperatively, the refractive errors was -0.5D/+0.25D × 98 with UDVA 20/20 and BSCVA 20/15 OD and -0.5D Sphere with UDVA 20/20 and BSCVA 20/15 OS. No intra- or postoperative complications were observed at 8 months postoperatively.
Conclusion/Take Home Message:
TICL placement combined with the SMILE procedure provided a safe and effective outcome for this very high myope. Our patient experienced not only improvement in UDVA and refraction, but she also experienced improvement in BSCVA. To our knowledge, this is the first published report of successful bioptics procedure combining TICL followed by SMILE OU.
