Abstract Details

Purpose:

Report a case of unilateral drop in vision caused by progressive hypermetropia due to an upper eyelid chalazion.

Setting:

Ophthalmology Department, Specialized Medical Center Hospital, Riyadh

Report of Case:

A 47 year-old woman presented with a distortion in vision in the left eye of 3 month duration. Uncorrected visual acuity on presentation was 20/20 OD, 20/120 OS, best corrected to 20/20 OD, 20/35 OS.Refraction on presentation was +0.50 D Sphere in the right eye, and +2.75 D sphere/ -0.50 D axis 140. Clinical examination showed a localized small central chalazion in the left upper eyelid. . Corneal Topography revealed flattening of the central left cornea. Symptoms resolved after incision and curettage of the chalazion and normalization of the corneal topography within a week of surgery with improvement in uncorrected visual acuity (20/20 OD, 20/20 OS), best corrected visual acuity (20/20 OD, 20/20 OS), refraction (+1.0, -0.25 X 15 OS), and high order aberration.

Conclusion/Take Home Message:

We report a case of hyperopia and astigmatism due to a flattening effect of small chalazion on the cornea, and reversal effect of chalazion surgery on the refractive errors and corneal topography. Chalazion effect on corneal parameters should be taken into consideration especially in dependent procedures like refractive surgery and cataract surgery.
