Abstract Details

Purpose:

The purpose is to show such benefits of the femtosecond laser as working on the closed eyes, the accuracy of the capsulorexis, and the reduction of the US energy during phacoemulsification on two complicated clinical cases

Report of Case:

In eyes with subluxated cataract , any manipulations on the capsule and lens are complicated by the weakness of the ligaments. Case1. Patient with the lens dislocation was treated on the femtosecond laser due to importance of the CCC exact diameter, centration CCC and the lens fragmentation on the closed eye. During the phacoemulsification the capsule bag was fixed by the iris retractor. IOL was implanted into the capsule bag. Surgery was competed without complications. 1 day after surgery UCVA 0,7. Case 2. For the eyes with corneal pathology it is very important to reduce the destructive effect on the cornea and decrease the US energy. Cataract eye with Fuchs Endothelial Corneal Dystrophy (FECD). VA 0,001, ACD 2,0 mm, LT 6,0 mm, FECD, ECD – 1481 cells/mm2. Femtosecond laser was performed. The lens capsule was removed without the use of the dyes so as not to cause additional toxic effects on the endothelium. Despite the large size of the lens it was able to reduce US energy, CDE = 6,04, due to the femtosecond cuts in the lens. BCVA = 0,8 first day after surgery. ECD = 1431 cells/mm2 after surgery.

Conclusion/Take Home Message:

These cases confirm the convenience and safety of using a femtosecond laser, where its advantages are important in obtaining the desired results and minimizing complications.
