Purpose:
To describe the successful management of malignant glaucoma after uneventful phaco surgery.
Setting:
Department of Ophthalmology, University Hospitals Leuven, Belgium
Report of Case:
A 60 years old man with high hypermetropia of +9.0 diopters (D) and very shallow anterior chamber depth (ACD) of 1.22mm (internal diameter, scheimpflug topography) underwent uneventful phaco surgery at his right eye. A 40 D SN60AT lens (Alcon) was implanted in the capsular bag. At day one after surgery, best-corrected visual acuity (BCVA) was 20/30 with -6.0D. Scheimpflug images showed an internal ACD of 1.77 mm. Intraocular pressure (IOP) was 14 mmHg. Cycloplegia was started. At week one after surgery, the IOP increased and the myopic shift remained. A full vitrectomy combined with iridectomy and zonulectomy was performed because of malignant glaucoma. After vitrectomy, the IOP normalized and the internal anterior depth increased to 3.35mm. BCVA measured 20/25 and the myopic shift was reduced to a spherical equivalent of -1.0D.
Conclusion/Take Home Message:
Postoperative management of phaco surgery in nanophthalmic eyes can be challenging.
