To report the effect of pinhole pupilloplasty for management of high astigmatism at the time of cataract surgery

Report of Case:
A 48 years-old female presented with complain of blurred vision for distance and near, halos around the light. She was wearing thick glasses. No history of trauma or previous ocular surgeries. On examination, her uncorrected visual acuities were 20/400 (right) and 20/200 (left). The best corrected visual acuities were 20/80 both eyes with the following refraction:
-13.50D -10.00× 5° (Right eye) and -9.00-11.00 × 170 °(left eye). Anterior segment examination showed bilateral nuclear sclerotic cataract. Dilated fundus examination showed tilted optic discs with peripapillary atrophy. Corneal topography showed 7.00 D of regular astigmatism both eyes suggesting the presence of significant corneal astigmatism. Pre-operative pupil diameter was 2.5 mm. The patient underwent right phacoemulsification combined with single pass four throw pinhole pupilloplasty technique. At a 1-month postoperative follow-up, the uncorrected visual acuity was 20/40 and remained stable over the following months.
Conclusion/Take Home Message:
Pinhole pupilloplasty combined with phacoemulsification for patient with high corneal astigmatism is an effective and reversible method as it significantly resulted in marked improvement in visual acuity and high patient satisifaction
