Purpose:
To report a case of 2 posteriorly dislocated foldable IOL's in the same eye managed by vitrectomy and explantation of one IOL and repositioning of the other.
Setting:
Ophthalmology Department, Sohag University Hospital, Sohag, Egypt.
Report of Case:
A 53 years old female, presented by drop of vision in the right eye after phacoemulsification done 45 days ago. On examination the eye was found to be aphakic with uncorrected visual acuity of counting fingers and corrected to 0.3 LogMAR by refraction of +9.5 DS & +2.75 DC at axis 55. Fundus examination revealed the presence of 2 dropped foldable IOL's. The case was managed by 23G vitrectomy under local anasthesia with explantation of one IOL and reposition of the other one on the reminants of the lens capsule with sufficient support.
Conclusion/Take Home Message:
The condition of two dropped IOL's is a newly reported complication of phacoemulsification and indicates malpractice of the first surgeon as he tried to implant a second IOL after drop of the first one without performing vitrectomy. To deal with a situation like this we can explant one IOL and reposition the other one.
