Purpose:
To present a case report of 60 years male diagnosed with Pellucid Marginal Degeneration and Senile Immature Cataract. Micro Incision Cataract Surgery was planned with customized toric intra ocular lens with BCVA 20/20 post surgery.

Report of Case:
A 60 years old male presented with slow and progressive diminution of vision for 6 months and history of wearing glasses for 45 years. On clinical examination he was diagnosed to have senile immature cataract and corneal degeneration confirmed as pellucid marginal degeneration on corneal tomography (Oculus Pentacam). BCVA both eye were 1.00 on LogMar chart with discrepancies in Pentacam and IOL master calculated keratometry reading and astigmatism.

Micro incision cataract surgery with customized toric intra-ocular lens was planned for the patient. His refraction post cataract surgery: OD: -0.5 Sph -1.00 Cyl at 60 degree; OS: -1.00 Sph -1.50 Cyl at 90 degree with BCVA both eye 0.00 on LogMar. Patient is under the follow up for over a year with stable refraction.
Conclusion/Take Home Message:
Intraocular toric lenses can be customized for the patients with irregular astigmatism due to corneal degeneration for visual rehabilitation and make them glass independent to a greater extend. Proper counseling and post surgery follow-up is the mainstay of managing such patients.
