Capsulorhexis in femtolaser age: a comparison between quality and safety

Abstract Details
Purpose:
A comparison between capsulotomies obtained by femtolaser-assisted cataract surgery (FLACS) with Catalys® Precision Laser System and LenSx® Precision Laser System and capsulorhexis obtained with manual technique in order to assess the presence of anatomical and functional differences between the three surgical methods.

Methods:
We evaluated the capsulorhexis of 150 eyes who had senile cataract. 50 eyes underwent capsulotomy femtolaser-assisted with Catalys, 50 with LenSx and 50 capsulorhexis performed manually using conventional technique of Continuous Curvilinear Capsulorhexis (CCC). All patients were operated by the same surgeon. The samples were evaluated using the Scanning Electron Microscope (SEM) to compare morphology, size and thickness of the edge of the capsulorhexis.
Results:
Capsulorhexis obtained with femtolaser were perfectly circular, whereas manual technique exhibited some alteration of the circular shape. Micro-incisures on the capsule's perimeter were observed on capsulotomies performed by means of femtolaser. Micro tears in the margin of section and abnormal pericapsular laser perforations probably caused by micro eye movements during surgery were also observable. These factors were not related to intraoperative complications. On the other hand, capsulotomies performed manually or with LenSX did not present abnormal perforations.
Conclusions:
The use of femtolaser in cataract surgery showed high standards of safety and quality allowing to obtain capsulotomies perfectly regular with better geometry and centering compared to those obtained with standard methods.
