Outcome of conservative treatment vs intraoperative air descemetopexy for inadvertent localised Descemet's membrane detachment during phacoemulsification surgery

Abstract Details

Purpose:

To analyse the outcome of intraoperative air bubble descmetopexy post phacoemulsification versus conservative management of localized descemet’s membrane detachment.

Setting:

University College London Hospital NHS Trust

Methods:

20 consecutive eyes of 25 patients with localized descemets membrane detachment during phacoemulsification were enrolled. First 20 eyes were managed conservatively and the subsequent 20 eyes were injected with intracameral 100% air bubble at the end of surgery.

Results:

All 20 eyes had phacoemulsification with IOL insertions. Successful reattachment of Descemets membrane and resolution of corneal oedema occurred in all cases. Mean time for corneal oedema resolution in-group 1 (conservative treatment) was 20 days versus 8 days in-group 2 (air bubble treatment). Best-corrected visual acuity (BCVA) at 3 months in-group 1 was 0.2 log Mar or better. In comparison, BCVA in-group 2 was 0.10 log Mar or better.

Conclusions:

Air descemetopexy appears to be superior to conservative treatment with regards to quicker visual recovery and achieving a better BCVA after localized descemets membrane detachment during phacoemulsification surgery.
