Tilted optic disc and its role in excluding keratoconus

Purpose:
The mainstay in diagnosing keratoconus is topography and tomography. There are some symptoms and signs that could raise suspicion of keratoconus and make the ophthalmologists order those tests. Observing tilted disc in fundus exam could be a simple way to exclude keratoconus and decrease in ordering for those tests.

Methods:
Keratoconus was suspected in 23 patients due to high cylinder, difference in cylinder between the two eyes or high keratometry. Fundus exam in all 23 patients revealed tilted disc. Pentacam (Oculus, Germany) was then ordered to exclude keratoconus.
Results:
Mean age was 17 years. The spherical equivalent refraction (SE) was –5.00. Mean keratometry (K) was 47.5. Pentacam tests with Belin/ Ambrosio Enhanced Ectasia map for all patients revealed no keratconus
Conclusions:
Tilted disc on fundus exam could be a simple and practical way to narrow the suspicion for keratonus and decrease the number of topography and tomography ordered for diagnosis of keratoconus.
