Abstract Details
Purpose:
To evaluate liquid hydrogel ocular bandages (OcuSeal™ and ReSure™) to cover and protect clear corneal cataract surgical incisions. To clarify how much hydrogel benfages applications as a standard for cataract surgery have improved wound watwertight properties . To discuss that only a problem of cost is a barrier to introduce those in high volume low cost cataract surgeries
Setting:
C.M.A. Centro Microchirurgia Ambulatoriale Monza, Italy
Methods:
At the conclusion of cataract surgery,we applied OcuSeal™liquid bandage(Beaver Visitec International) to 568 eyes of 377patients(mean age 67.44 ±11.98) or ReSure™Adherent Ocular Bandage (Ocular Therapeutix Massachusetts,USA) to 74 eyes of 65 patients (mean age 68.11 ± 9.89) to reinforce closure of clear corneal incisions (up to 3.75 mm size).OcuSeal™consists of a synthetic dentritic hydrogel comprised of 2 separate components:powdered polyethylene glycol(PEG)and liquid polyethylene amine(PEI). Resure™ consists of a polyethylene glycol-based hydrogel.Hydrogel is activated by mixing(for ten seconds) 2 separate components:powder polyethylene glycol (PEG) and a diluent liquid precursor containing trilysine. After application, both bandages cross-link within 30 seconds
Results:
During the immediate post-operative interval, patients treated with both bandage reported improved comfort. Resure™ protected the wound by remaining localized over the de-epithelialized tissue for at least 48 hours in 74 eyes (100%) of patients while Ocuseal™ within 12 hours.
Conclusions:
Liquid hydrogel bandage applied at completion of cataract surgery enhances incisional wound closure and improves post-operative patient comfort. After refilling with BSS, the anterior chamber depth is increasing with the bandages due to improvement of wound watertight properties. Hydrogel bandages are helpful to avoid liquid leakage from AC in both directions. This is a positive option to reduce endophthalmitis rate. Surgeons have to be confident with hydrogel bandages use to avoid lost of efficiency. Discontinously
